Individual Plans Provider Directory

If you wish to apply by mail, choose your plan and follow these easy steps :

1. E-mail for a paper application.

Important: Receipt of your initial payment does not constitute enrollment in this program. Your coverage does not begin until your application has been approved and you have been assigned an Effective Date of Coverage.

2. Include your First Premium Payment. Premium payment for applications can be paid via ACH (Automated Clearing House) or with a check.

• If you elect to pay electronically by ACH, you must authorize one month's premium by submitting a completed ACH authorization form.  Please include a voided check (for checking account withdrawals) or a deposit slip (for savings account withdrawals) with your application.
   

• If you elect to pay by check, HMO applicants should make checks payable to Keystone Health Plan East. PPO applicants should make checks payable to Independence Blue Cross.

3. Return your completed application to:
